For 2011-2012 the Staples Athletic Department will offer a very competitive and comprehensive program for all students. The Interscholastic Program includes 65 different teams. Visit the Athletic Teams page for team websites, coach and contact information, and tryout dates.

ATHLETIC FORMS

Requirements for Student Participation in the Staples Interscholastic Sports Programs, Including Tryouts

We hope this information will assist you in understanding our forms and process, clarify the requirements, and help you and your student athlete complete the forms according to specifications so that they will be eligible to participate in tryouts, practices, and competitions throughout the sport season.

1.      Annual health history and physical examination (NEW HAR -3 Rev 2010):  Every athlete must have on record a current health history and physical examination before he/she can participate in a sport and throughout the sport season, including post season championship competitions. “Current” means that the physical examination and history have been completed within the immediate past 12 months.  Please use the NEW HAR-3 Rev 2010 to fulfill this requirement. 

a.      Part I - must be completely filled out and signed by the parent/guardian.  

b.      Part II – Medical Evaluation-- must be filled out completely and signed by the student’s physician.

c.      Part III – Immunizations – Updated as new immunizations are given.

d.      The physical examination is valid for 12 months from the date of the exam.  For example, an exam completed by the physician on October 20, 2009 will be valid until October 20, 2010. If the student does not hand in a HAR-3 Rev 2010 on or before October 20, 2010, he or she will not be able to participate in sport practice or competitions after November 20th until such time as the athlete hands in a new (current) properly executed HAR-3 Rev 2010.  If timing of the physical examination is an issue due to health insurance restrictions, please contact one of us immediately for assistance.

e.      This form may be handed into the school nurses or to Gaetana “G” Deiso, the Head Athletic Trainer.  One of the school nurses must review it before the student will be medically cleared for participation.

f.       Please note: We will continue to accept the old Staples High School Form A and B through June 2011. Your student athlete will not be eligible to participate in the athletic program without either an old Staples High School form A and B or the new HAR-3 Rev 2010 form on file.

g.      There are no exceptions to this policy.

 

  1. Seasonal Sport Permission/Emergency Information (Form C):
    A Form C needs to be completed and handed in for each and every sport the athlete plans to try out for EACH sport season.  Form C is a carbon-copy GOLD form that includes emergency contact information and permission for emergency medical treatment in the event that a student is injured at an away game and needs to be transported to a medical facility.
    1. This form should be “verified” by the Athletic Trainers or the school nurses.  The coach will receive the top copy of form C and the bottom copy will reside with the Athletic Trainers. 
    2. These forms will NOT be accepted if they are handed in to a coach or person other than those designated above.  There have been too many errors, including lost forms and delays in student participation, when forms are not brought to the proper place for review and processing.

Please note we will need 2 copies of Form C if it is downloaded. Both forms must be filled out using a pen. A form that is filled out in pencil will not be accepted.  If you complete the forms and turn them in as described above, your student athlete should have a smooth start each sport season.

Forms can be mailed or handed in at the Health Office, not the Athletic Office.  During most days in August, a health assistant and nurse will be in the Health Office to receive forms, but because of schedule variability, it is best to call before coming. Enjoy the summer and please complete and return or mail the forms for fall sports as soon as possible.

 

Athletic FAQs

What forms do I need in order to participate on an athletic team?
The state has issued a NEW 3 page blue form (HAR 3) that includes the criteria needed to participate in the athletic program. Local pediatricians’ offices will have this form in their offices or you can get it on this website or in the athletic or nurses’ office. The form is also available at http://www.sde.ct.gov/sde/lib/sde/PDF/deps/student/health/HAR.pdf. The old Form B will NOT be accepted for the 2011 – 2012 school year. Form C is an Emergency Information form to be completed by the parent. Both forms must be approved by the nurses or athletic trainers before an athlete can participate in practice or games.

Can I give the forms to my coach?
No, they must be approved and stamped by the nurses or athletic trainers before an athlete is eligible to participate.

My physical expired and I can’t get an appointment for another month. Can I get a note from my doctor and still be able to participate until I have my physical?
No, only the
new blue HAR-3 form will be accepted. No exceptions.

What is the time commitment to belong to a team?
Teams practice Monday – Friday and sometimes Saturdays depending on the sport, team and level.

Can an athlete tryout for more that one sport during a single season?
No, each athlete may only try out for one sport during a given season, however, if they are cut from a sport that has a tryout, they are welcome to join a team that does not have a tryout, such as track and swimming.

Can a parent drive me to or from a game?
Yes, a
Travel Release form needs to be completed and signed by the parent, the athletic director and the principal. Once the form is completed, it is to be given to the coach. A parent may only transport their own child.

Can students drive to or from a game?
Students are not allowed to drive to or from a game.

How do I find out about last minute cancellations?
Call the
Athletic Hotline at 203-341-1333. Do not leave a message on this line.

Who do I call if I am going to miss practice or a game?
Coaches have their own team policy regarding absences that involve contacting your coach directly. Do not call the Athletic Department.

I need to talk to someone about my PE class, who do I call?
The Athletic Department does not oversee PE classes. Contact your PE teacher or the PE Department Chair.

Where can I find directions to an away game?
Refer to your Athletic Handbook or visit the Athletic link on the Staples High School website and click on
Directions.

How can I find my team’s schedule?
Go to
www.ciacsports.com. Scroll down to View Game Schedules. Select Staples from the drop down list of schools. Choose V (varsity), JV (junior varsity) or FR (freshman) from the drop down lists of levels. Select the sport from the Sport/Activity drop down and click View Schedule. This schedule is subject to change, so check it periodically.

What happens if I lose my uniform or equipment issued by the school?
Uniforms and equipment are issued to athletes for a season. Once the season has ended, it is the responsibility of the student to return the uniform to their coach. Unreturned uniforms and equipment will result in an accountability which will restrict access to the Home Access Center preventing access to report cards. The Athletic Department will determine the replacement cost of lost uniforms and equipment. Once uniforms and equipment are returned or paid for, the accountability will be removed.

How is the Staples Scholar Athlete chosen?
The Staples Scholar Athlete Award is given to one senior athlete from each of our varsity programs. The criteria for selection include outstanding athletic accomplishments, excellence in the classroom, strong character, leadership on and off the field and valuable contribution to the community. The award winners are nominated by their coach and are honored at a banquet held in June.

Does the Athletic Department have a website?
Yes, it can be reached through The Staples High School website. Most
teams have their own websites that are managed by the teams.

For students interested in less competitive sports, the Staples Athletic Department offers a very popular intramural program. These activities are offered to all students regardless of ability. A fully operational fitness center is available after school for all SHS students.
